If you use the callback form, we receive the name and phone number you enter and the topic you select. We use them only to contact you about your request. Please do not enter medical details in website forms.

When you call or text us, your phone carrier connects you to our office line. Online booking is provided by Square, and information you enter there is handled under Square’s privacy policy.
